@NMNeurology On the flip side, how should we be treating patients with Late Onset MS? The 50s are still young and should we be sacrificing efficacy for fear of lymphopenia, hypogammaglobulinemia, infections, PML? Perhaps a short course of immune reconstituting therapy is the answer.

Daniel Jons (Gothenburg) looked at 669 pre-symptomatic case control pairs in Swedish Biobank and found sNfL elevated 5-10 years before MS and auto reactive EBNA1 10-15 years before MS. There was no elevation in sNfL in EBV neg patients (though this was a younger group).

What causes MS? EBV remains a plausible instigator, but why does MS happen years after infection and why are there no EBV proteins in the CSF of MS patients? Peter Stys (Calgary) found a unique amyloid in MS brains... maybe this is triggered by EBV #ECTRIMS @NMNeurology

@gavin_sowa used PhiP Seq with Michael Wilson and found autoantibodies to a serine-arginine repeat motif present in CSF years before MS disease onset. He also found elevated sNfL levels prior to MS symptom onset with Ari Green. More evidence for a prodromal state.

What risk factors increase risk of #RIS converting to #MS? Age <37, enhancing lesions, spinal cord lesion, infratentorial lesions & oligoclonal bands. 3 factors: 91% chance of developing MS in 2 years. #ECTRIMS2022
