AC: yes. in direct answer to your question I consider heart rate of 181 per minute to be very concerning.

AC: I dont think that I, well I am sure, I was not aware of Coco's sustained pronounced tachycardia in the hours leading up to my assessment

AC: I do not remember looking at Coco's observations chart, records which are electronic, which record her temperature, pulse, respiratory rate. It is my normal practice to do so and I don't believe I did so before I saw Coco.
