After removing the bone flap, the hypertrophied posterior branches of the middle meningeal arteries can be visualized, leading to the center of the tumor. Adequate epidural hemostasis is secured. The dura is opened around the base of the tumor, and all affected dura is resected. Again, the edges of the dura are coagulated to reach hemostasis. The affected piece of dura is mobilized in the middle of the resection cavity. An important operative maneuver is the use of long cottonoid patties to mobilize the tumor, away from the brain starting with the arachnoid planes between the tumor and the surrounding surfaces. You can see that the maneuver of mobilizing the tumor, and dissecting the planes at the tumor parenchyma interface is quite useful for gaining full exposure around the tumor.
