An extraordinary incident today onboard a Ryanair (Malta Air) Boeing 737-800 - the jet suffered an engine failure around 10 min after takeoff, and debris struck a cabin window, causing rapid decompression. The passenger in the window seat was reportedly pulled partway through the opening by the pressure differential, before being pulled back in to the cabin by other passengers. Thankfully everyone is safe & aircraft returned as an emergency landing.

Before we get to the nitty gritty, the process is flawed. The entire investigation sits on the data that can be retrieved from the flight recorders.
1. After the 2nd FDR was found, the recorders sat in AMD from 16-24 June with a thousand B787 flying.
2. The equipment to download data was sourced from @NTSB on 25th which is when the FDR were shifted to Delhi lab. There may be a claim that certain portions of data could not be retrieved.
3. Passengers may have recorded the takeoff on their phones. The families were returned the phones but after being smashed with a sharp object.

1. Major fatal crashes need independent inquiry.
2. Once formal inquiry is ordered, earlier investigator should not give competing interpretation.
3. A highly experienced pilot must be associated where needed.

In 2020 at Calicut they couldn’t reach the crash site on time.
In 1990 Bangalore crash they couldn’t open the perimeter gate.
