• “Will continue to monitor.”
• “Patient resting comfortably.”
• “Patient resting in bed with eyes closed.”
• “No acute distress.”
• “Stable.”
• “Continue treatment as ordered.”

INSTEAD SAY:👇🏾

• Plan of care ongoing.
• No new concerns at this time.
• Patient observed resting in bed with eyes closed. Respirations even and unlabored. No distress noted.
• Patient reassessed. Findings unchanged from previous assessment unless otherwise documented.​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​

Remember that one day your chart may be audited
