@JamesTitcombe Ideally, enablers, behaviors and outcomes should be assessed, with a emphasis on enablers. Most tools only assess one of these dimensions. A combination of different tools should be considered from my point of view.
@PatrickWaterso1 I really like this idea that aviation isn’t a paradigm. Other industries also do safety and in various ways. Healthcare should not only look to aviation. And also cautiously transfer concepts from other industries in general.
