In the US there is no concept like Arbeitsunfähigungsbescheinigung (lol german) - you just tell your boss that you are sick, and you are good to spend a few days resting. If you abuse it -- you are fired.
In Germany if you have the sick leave paper you are legally can stay home for 3 days unlimited amount of times (yes every week) and no one has legal right to fire you.
