People move around so why do their detailed medical records almost never follow them?

Starting from scratch with people who’ve been in secondary services for years makes for poor care

@nuwandiss I would not say this is just a problem in psychiatry. I have moved around a lot and I live with a chronic physical illness. My physical health records have not followed me. My mental health records have not followed me either
