A 78-year-old man with undergoing transfermoral TAVI had embolization of a balloon-expandable valve into the descending aorta. Initially stable, he developed acute paraplegia the next morning due to delayed valve twisting with near-occlusive aortic flow and spinal hypoperfusion. Emergency valve-in-valve implantation restored flow, with complete neurological recovery within one week.
