I'm a cardiologist. Here's why I recommend men take 5 mg of tadalafil — Cialis — every single day.
Not for ED. Not for performance.
I take it for the same reason every serious longevity physician I respect does: to protect my cardiovascular system, my brain perfusion, and my endothelial health at the most fundamental level.
This drug — famous for all the wrong reasons — has quietly become one of the most powerful tools in preventive cardiology. And the data is now too strong for me to keep quiet about it.
I am not against SACCOs. They work well for many people, especially those seeking disciplined savings, dividend income, and access to relatively affordable credit. However, personally, I have never been a strong fan of the SACCO model in Kenya and I have never joined any!
Here is why:
1.Why should I write a withdrawal letter and wait 60 days or more just to access my own savings?
Most SACCOs impose notice periods before refunds are processed, especially for non-withdrawable deposits. While this is meant to protect liquidity and the stability of the SACCO, it significantly reduces financial flexibility for members.
2.Why is it that if I accumulate share capital worth Ksh 1 million, I may not freely access it unless another member agrees to buy or transfer the shares from me?Why can't I sell them or why can't the sacco pay me and absorb them?
Unfortunately, in many SACCOs, share capital forms part of the institution’s permanent/core capital and is generally non-withdrawable. In most cases, it can only be transferred or sold to another member upon exit.
3.Why should I borrow “cheap loans” against my own savings instead of simply withdrawing my savings?
The SACCO model is designed around leveraging deposits to create access to credit rather than allowing unrestricted withdrawals. This works well for members who want long-term disciplined savings and credit access, but for liquidity-focused investors, it may feel inefficient.
Why do I need guarantors to borrow money that is already secured by my own deposits?
Many SACCOs require guarantors because loans are issued based on cooperative risk-sharing principles. Members often qualify for loans worth two to three times their deposits, and guarantors act as additional security to protect the SACCO from defaults.
From a purely liquidity and capital-efficiency perspective, it may not always make financial sense to pay interest on a Ksh 300,000 loan when you already have Ksh 600,000 locked in deposits that are not easily accessible

1️⃣ Overview
Stroke refers to an acute neurological deficit caused by a disruption in cerebral blood flow, categorized into ischemic (∼85%) and hemorrhagic strokes.

2️⃣ Initial Assessment
➊ Immediate Actions (DO):
🔹 NCHCT (Non-contrast Head CT) – rule out hemorrhage
🔹 Labs: Glucose, CBC, PT/PTT, troponin, creatinine
🔹 Fingerstick glucose: If <60 mg/dL → treat for hypoglycemia
🔹 Evaluate for signs of intracranial hemorrhage (ICH)
• If ICH suspected, assess for coma (↑ ICP) or severe headache (suggestive of SAH)

3️⃣ If CT shows no hemorrhage → Suspect Ischemic Stroke
➊ Acute Management (Based on Timing):
🔹 <4.5 hours since last normal
• Evaluate tPA criteria → Administer IV tPA
• Maintain BP <180/105 mmHg
• If large vessel occlusion (LVO) → consider mechanical thrombectomy
🔹 4.5–24 hours since last normal
• If LVO present (on CTA/MRA) → mechanical thrombectomy
• No tPA unless part of extended window trials
🔹 >24 hours since last normal
• No acute intervention → Begin secondary stroke prevention
🔹 TIA (Symptoms resolved)
• Manage with permissive hypertension, glucose control, and etiology-based prevention

4️⃣ Ischemic Stroke Workup – Identify Etiology
➊ Thrombotic Stroke
🔹 Due to atherosclerosis or large artery stenosis/dissection
🔹 Tests: CTA/MRA head & neck, ECG, echocardiogram
🔹 Treatment: Aspirin ± Clopidogrel
• Carotid stenosis >60% → Consider carotid endarterectomy
• Refer to hematology for hypercoagulability testing if indicated
➋ Cardioembolic Stroke
🔹 From atrial fibrillation, cardiac thrombi, or reduced EF (<30%)
🔹 Seen on echocardiogram or ECG
🔹 Treatment: Long-term anticoagulation
➌ Venous Embolic Stroke
🔹 Often due to paradoxical embolism via PFO
🔹 Consider bubble study + evaluate for VTE risk
🔹 Requires hypercoagulable workup and possible PFO closure

Those deep painful bumps and clusters come from internal stress signals, especially:
• Gut irritation / endotoxin overload
• Hormonal imbalance, especially high estrogen, serotonin, cortisol and histamine
• A liver that’s overwhelmed and can’t fully clear those hormones
• Low metabolic / thyroid function which slows healing and increases inflammation
When the gut leaks endotoxin into the bloodstream, the immune system responds aggressively and the skin (one of the first places inflammation shows up) reacts with swelling, redness, clogged pores and cyst-like breakouts.
This is why topical treatments alone rarely solve severe acne: the skin is a mirror of inner stress, not the source of it.
What actually helps
A. Calm the gut: this is the biggest lever for clearing skin
The fastest-acting gut tools for acne:
• Daily raw carrot salad (grated carrot + coconut oil + vinegar and salt)
—> binds endotoxin and estrogen, lowers inflammation, improves bowel movements
• Avoid foods that bloat you (they feed bacteria behind the breakouts)
—> e.g. beans, oats, raw leafy salads, excess starch, nuts/seeds
• Eat easy to digest carbs: ripe fruit, orange juice, honey, well cooked roots
• Cook your vegetables well or choose low-irritant ones (mushrooms, zucchini)
• 1 bowel movement per day minimum
—> magnesium glycinate/citrate or cascara sagrada can help safely
• Coconut oil (1–2 tsp/day)
—> antimicrobial, pro-thyroid, reduces endotoxin
Lower gut inflammation —> lowers estrogen, serotonin, histamine, cortisol —> skin rapidly calms.
B. Support metabolism and liver function. A strong metabolism = strong skin healing
• Get enough protein (80–100 g/day)
Best sources: dairy, eggs, gelatine, shellfish, white fish, beef/lamb; 150g of liver a week
• Avoid all PUFA oils
—> no seed oils, vegetable oils, nuts, nut butters, margarine, avocado oil, fish oil, fatty fish
They slow metabolism, increase inflammation, worsen acne, just cut off
• Eat saturated fats instead: butter, coconut oil, dairy fat, tallow
• Don’t undereat because calories help the liver detox estrogen
• Sunlight / tanning shop daily to raise vitamin D and lower inflammation
• Sleep 8–9 hours —> cortisol drops, skin heals
C. Balance hormones. Most severe acne involves estrogen dominance + high cortisol.
What lowers estrogen safely:
• Raw carrot salad (again, it’s huge)
• Adequate protein + low-PUFA diet (crucial!!!)
• Good thyroid function (all my tips will help you with this)
• Daily bowel movements
• Avoiding soy and excessive alcohol consumption
What raises protective hormones:
• Orange juice / fruit sugar
• Dairy
• Enough calories
• Saturated fats (esp. coconut oil)
Supplements:
Niacinamide (Vitamin B3): 50–100 mg, 2× per day
• lowers inflammation
• supports metabolism
• decreases cortisol
• helps “oiliness” and skin healing
Magnesium glycinate: 200–400 mg nightly
• helps bowel movement regularity
• lowers stress hormones
• supports sleep
Vitamin E (mixed tocopherols): 100–200 IU/day
• anti-estrogen
• anti-inflammatory
