Common extensor tendon tear of the elbow.
Maximum elbow flexion is extremely useful in better identifying tendon tears.
The tears usually have a longitudinal orientation; elbow flexion compresses the tendon like an accordion, opening these longitudinal tears.
"BUT," if you see that it opens up a lot with anechogenic fluid, you must have a high suspicion of joint communication. So try to find it!

Highlighting an interesting case by @JakeSellonMD and myself demonstrating an impressive myositis ossificans within the conjoined tendon and biceps femoris.
The preliminary diagnosis was made in the training room by no other than @BrianSuttererMD during his fellowship!
Attended my 6th #AMSSM conference, and the last one as a trainee!
Such a wonderful organization to learn from great minds, share new ideas, and catch up with old friends.
Looking forward to continuing these efforts as an attending!
