@orchid_grey But I think a big part is, *even if* eventually somehow they decide the denial was wrong, there's no disincentive to deny. All they pay is what they should have paid from the outset. So, may as well deny everything and hope people give up.

Every time I've been denied a claim, it's been due to the company's "internal policy" which is both opaque to me, and unrelated to reality. And I'm left in a cycle of appealing and being denied with no recourse.
