Not holding the responsible party accountable is unacceptable! It’s a trend I see within teams & departments. Whenever we say something to the effect of “we” need to fix this or “we” need to fix that…it’s not holding the person accountable. Sure, “we” will suffer if that person doesn’t fix the problem but they need to be held accountable on an individual level.
