An AI agent deleted a startup's entire production database in just 9 seconds, then confessed in writing to ignoring all its safety rules.
Jer Crane, founder of @pocketosai a platform that helps rental businesses manage reservations, payments, vehicle tracking, and more recently shared a major incident. His team used cursor_ai 's flagship @claudeai opus 4.6 agent, equipped with explicit safeguards, to fix a routine staging credential problem. The agent instead deleted a Railway production volume via API, erasing all data and backups. Railway keeps backups in that same volume, leaving nothing recoverable.
The agent explained itself: "NEVER F****** GUESS!" and that's exactly what it did. It guessed without verifying, executed a destructive action without instruction, and violated every safety rule provided.
Cursor's Destructive Guardrails and Plan Mode failed to intervene. Railway's API lacked confirmation steps, CLI tokens granted full root access across environments, and the backup setup offered no real protection.
More than 30 hours later, Railway provided no recovery option. PocketOS customers manually reconstructed bookings from Stripe records and emails over a busy weekend.
The case reveals how AI infrastructure deploys agent tools like Railway's MCP faster than safety measures. Agent prompts alone do not ensure reliability.
AI builders should audit token permissions, backups, and testing setups immediately.

Vercel’s this $9.3B beast powering Next.js (6M downloads a week) and millions of live sites.
Their CEO Guillermo Rauch called the attackers "highly sophisticated," maybe even AI-boosted. But get this it wasn’t some zero-day magic.
Just one employee messing with a third-party AI tool called Context AI.
That tool had Google Workspace OAuth access. Tool got hacked, attackers owned the employee’s Workspace, then waltzed into internal env vars (the "non-sensitive" ones), grabbed 580 employee records to flex, and bounced.
Real talk, founders: every AI tool your team’s clicked "allow" on for Gmail, Drive, Slack, CRM? That’s a backdoor with your name on it. No idea how many you’ve got? Yeah, you’re probably screwed.
