Today is #RareDiseaseDay #Awareness

Clinical trials are essential for developing treatments, overcoming research challenges, and ensuring a patient-centered approach.

I would like to share a brief summary I wrote about the challenges & solutions:

Challenges in Rare Disease Clinical Trials:

- Small patient populations

- Limited understanding of disease progression

- Lack of established treatments: Placebo-controlled trials may be ethically challenging.

- Regulatory hurdles

Role of Clinical Trials in Rare Disease Research:

Clinical trials for rare diseases follow the same phases as other drug trials but often use innovative designs to address the challenges:

- Natural history studies: These help define disease progression, identify biomarkers, and determine meaningful endpoints.

- Adaptive trial designs: Such as Bayesian approaches or sequential trials that allow modifications based on interim data.

- Basket and umbrella trials: Used when the disease has genetic variations or subtypes that can be studied together.

- N-of-1 trials: Single-patient trials that provide individualized insights, particularly useful in ultra-rare conditions

Patient-Centric Approach in Rare Disease Trials:

Given the small and vulnerable patient population, patient-centered clinical research is crucial:

- Early Involvement of Patients and Advocacy Groups

- Decentralized and Remote Trials

- Expanded Access Programs (When a treatment shows promising results, compassionate use programs may allow patients access before full approval. Also some trials integrate early access models to ensure continuity of care post-trial.)

- Digital Health & Real-World Data Collection (long follow-up studies/ registries)

Nurses and midwives are dear to my heart and high on my agenda. I was delighted to join the Global Partners Meeting on Nursing and Midwifery today.

Our Region accounts for almost 18% of the global shortage of nurses, and if current trends continue we will lack 1.2 million nurses by 2030. We need to empower them and make sure we are able to retain them especially in countries affected by emergencies.

One of my flagship initiatives aims to build a regional health workforce that is agile and prepared to confront a changing public health landscape. This includes the education and employment of public health professionals, and the investment in their profession.
