31歳の男性で、交通事故による重度の脳損傷から3年以上にわたり植物状態と診断され意識が回復することはありませんでした

ところがゾルピデム10mgを投与すると、約20〜30分後から反応が現れ、家族や医療スタッフと意思疎通ができるようになりました

母親や医療スタッフと会話ができたり、簡単な計算や、食べ物を自分で口に運ぶ、冗談を言えるまで回復するといったことが可能になったと論文では報告されています

しかしこれは恒久的な回復ではありません

ゾルピデムの作用が切れる約4時間後には再び元の重度意識障害に戻りました

しかし、翌日に再びゾルピデムを投与すると、また4時間覚醒するという現象が繰り返されました

この現象はゾルピデムの逆説的覚醒と呼ばれ、彼だけでなく他の重度の脳損傷患者でもゾルピデムによる逆説的覚醒の症例研究はあります

2022年のレビューゾルピデムを慢性意識障害患者に系統的に投与した研究は数十〜100人規模まであり、再現性のある明確な逆説的覚醒を示すのはおよそ6％前後と言われています
