In intensive care, haloperidol, a calming drug, did not clearly help patients with delirium (sudden confusion over hours or days).
The AID-ICU trial (New England Journal of Medicine, 2022) gave 1,000 intensive care patients with delirium haloperidol or placebo (a dummy drug), chosen by chance.
The main measure was the number of days, out of the next 90, that patients were alive and out of hospital. The two groups did not differ clearly.
A second measure was deaths within 90 days: 36 in every 100 patients on haloperidol, against 43 in every 100 on placebo. The trial was not mainly designed to test this, so it is weaker evidence.
Intensive care colleagues: has this trial changed how often haloperidol is used in your unit?
