@DrToddLee@aclong111 Then you get to be a specialist and you realize there aren’t really answers to some of these questions and the dosing of bactrim is based on a case series of something like 12 leukemic children from 40 years ago and there’s no prospective data to guide us…
@nickmmark Seen a couple cvc studies where complication rates are higher than I would expect. This one found 2-5% rate of bleeding requiring surgical/IR intervention or resulting in death. That’s a lot higher than I thought even for this population w/ TCP

@BradSpellberg@DrToddLee@ABsteward For MSSA bsi beta-lactams are the standard of care for good reason though. I think this gets to the heart of hesitancy with mssa bacteremia which is you don’t have an oral beta-lactam with excellent pk/pd and a reasonable dosing schedule (cefadroxil closest I guess).
BREAKING: Kamala Harris just released this devastating ad slamming Donald Trump for having a problem with the truth. All Donald Trump does is lie, and when he’s not lying, he’s rambling incoherently. Retweet so all Americans see this.
@dpfeifleMD@gbosslet@emily_fri Anything in those studies to control for the circumstances under which PICCs are placed? I’m all for learning to place them, but I wonder if the effect is partially due to them being placed in generally more stable patients and more controlled conditions
Can somebody please explain why hospital credentialing is the most painful, time consuming, inefficient thing of all time? How can it take almost a year to get credentialed at a hospital? It’s actually comical.
@A_Sapp12 Always impresses me when an early trainee is thinking ahead a lot.
I’ll also echo what others have said — make sure the plan is actually getting done. Escalate if you need to.
Very appreciative when trainees take time to update patients and families thoroughly
@A_Sapp12 Sounds cliche but try to be proactive as much as possible and always anticipate worst case scenarios — if this bleed opens up what are we gonna do and in what order? If this angioedema progresses what are we gonna do and what equipment do we need and is it here?
Do you care for sick patients that need phenotyped resuscitation? If so, you can't miss this 👇
@ArgaizR and I are taking #hcprounds to the next level and are launching our website (https://t.co/xjhgsFUWw9) and newsletter where we explore the cutting edge of
hemodynamics, venous congestion and perfusion assessment.
Why? Because a 1-size-fits-all approach fails our sickest patients.
But... to make this a success, we need your help (only two things!) ! 🆘👇
1) If you haven't already, subscribe to our newsletter (https://t.co/AdJdNftGOY)
2) retweet this so we can start building a community dedicated to improving the care for our sickest patients 🙏

@alegalnerd It’s such an absurd position to take. How do you argue that with a straight face? “Ok, all presidents can try and have fake electors sent, no threat or punishment.” Absolutely rediculous
@alegalnerd listening to the current Supreme Court procedings — why has no one asked this obvious question — if we assume Trump’s teams position that the former president can’t be prosecuted for attempting to send a false slate of electors because it was an official act…
@alegalnerd Yes but IIRC Sauer didn’t concede the act regarding the fake electors; he referenced actions taken by Grant to say that was an official act. They should’ve hammered him on this, asked him if Biden did the same thing would he enjoy immunity? I would’ve loved to see him try &answer
@alegalnerd Yes but why not be specific about these allegations. Make Trumps attorney say out loud either it’s ok for Biden to steal the upcoming election this way, or that no one should get immunity for this and we’re all listening to a bunch of hypotheticals for no reason
@ProfGrewal@alegalnerd@EWess92@HumphreyBohun Yes, ACB went through the allegations one by one and he agreed that basically all were private acts. Why is this case even brought then?
What’s the deterrent for the current sitting president or any other future president from attempting the same thing? Why shouldn’t Biden go to every potentially sympathetic state legislature and ask them to send “alternate” electors. Is allowing that good for our democracy??!
