The Fourth of July is about celebrating the big, bold, inclusive experiment that is our American democracy. And it has always been an experiment. Our democracy has never been guaranteed, which means we can’t take it for granted. We need to keep fighting for it, keep improving it, and keep making sure it reflects the better angels of our nature instead of the worst. That, more than anything, is what America is all about.

We need many more of these. Increased employment of advanced practitioners everywhere is a priority combined with a focus on wrapping shared capabilities around the patient/citizen and drawing on the complimentary expertise of all our health and care professions.
This year NHS England gave practices just 1.9 per cent extra cash to cover all their expenses. The BMA is balloting GPs on industrial action that could severely affect NHS services. Labour has promised to make primary care a priority.
