@KprasMD@drmcumming OBLs not set up to manage urgent pts c unknown problems. busy OBL taking add ons undermines benefit and goal of OBL. hard 4 dr to interrupt day to spend enuff time to fully eval pts c uncertain issue. can't even usually get labs. delays all other pt procedures. er made for this
@KprasMD@drmcumming overstatement? @drmcumming had good outcome, but to send pt c possible lethal bleed to a hospital that can handle is not lunacy. what if bleeding could not be stopped? what if pt became unstable and needed pressors or blood or nephrectomy? clots in bladder, needing irrig?

@JacobFlemingMD thinking not enough. what is pathophys of steroids causing avn? do steroids injected into joint cross cortical bone? if it gets absorbed, does it enter artery or vein? what is the cause of avn? real doctors need to understand pathophysiology for pathology they treat
@IR_Doctor why is this a problem? i have referred biopsies to ir with something similar. pet report says hypermetabolic lesions, ddx offered, "consider bx". why can't the radiologist use their judgement to choose the best lesion to bx? need referrer to tell you exactly what to do?
@jbcarmody wise words about the value of research, coming from a community doc in rural virginia, who performs no scientific research, perhaps doesn't really know how to perform research, or tried and failed to publish actual science. ALL HEED!! ps-nothing wrong with being a great clinician
@jbcarmody many do 1st research project in training due to a requirement. some actually find they are good at it and love it and go on to pursue academics. many dislike it or are bad at it, and go on to private or community practice. won't know if you don't try. y criticize? sour grapes?

ER docs, surgeons, internal med specialists, hospitalists, nurses, techs work overnight, because diagnosis and treatment sometimes cant wait. why not radiology?

@GiaFugax@thats_bone@CSWhiteMD 3rd and most 4th years are faaaar from being capable doctors. yes they can help with tasks but they require teaching and supervision, which is much work for residents and attendings.
@CSWhiteMD i am team "let students experience what it is like to actually work in a given specialty as a resident and or attending", so that they know what kind of lifestyle said specialty entails. would decrease miserable doctors who didn't know what they were getting themselves into
