Another excellent book is "The Comporehensive Handbook of Constructivist Teaching" by Jim Pelech. The book addresses all disciplines, but Jim is a math professor and math teachers often have difficulty finding helpful information on constructivist practices in math.
A book to read if you are interested in constructivism, learner-centered practices and/or school reform:
"Schools Reimagined" is a follow-up to the widely read "Case for Constructivist Classrooms (1993, 1998) by
Jacqueline Grennon and Martin G. Brooks.
