only a wk after asco and we get a glimpse of another likely breakthru for pancreatic cancer patients. tango reported that in 39 mtap deletion patients (~40% of pancreatic cancer), adding tango's vopi to revmed's daraxon or zoldon doubled the response rate u typically see with rasi alone to 64%. the daraxon combo was especially jaw dropping, with 92% n=11/12 of patients responding. the waterfall is esp pretty, only 1 patient's tumor didn't shrink. amazing leap from chemo's ~10% response rates.

here’s how real life trials work. take cancer. assume a drug reduces death by half (very few drugs can do that). lets say patients die in 1yr. u will want to enroll 3-400 patients, that’ll probably take 2yrs and then u’ll wait another yr for data. most cancer trials today enroll more bc 30% is often viewed as really meaningful. this works for most cancer bc it is common. now just think thru a rare disease. can’t or takes yrs to enroll that many patients. what if the disease kills u in 10yrs? trial takes a decade. k find surrogate endpoints, but what’s a universal endpoint for a disease with little data bc of how rare it is (no tumor to shrink here), might take a couple decades of failed trials just to find a good endpoint. that is the real journey patient advocacy groups and drug developers have lived, many over decades. many experienced and caring regulators lived it with them and it led to the natural conclusions - flexibility based on common sense - single arm trials, surrogates, real world evidence. real example - some enzyme replacement therapies for lysosomal storage diseases are in fact cures, completely restore normal functioning. despite that because those diseases kill u over 10+yrs several 100 patient trials failed on the best endpoints people could come up with - often how far kids could walk. imagine asking a kid to walk and then comparing that 6-12mos later. kid is tired that day, doesnt want to listen, sick etc. now imagine if the disease affects the kids brain and there is lower iq. imagine if some kids walk fine but others are in wheelchairs. this is real life in drug dev. i went to ivory tower schools, took plenty of stats (and now teach it sometimes), but u only learn judgment in real life.
