@ciirarref Where I used to work, we had a professionalism committee. The clerkship and eventually the committee should be aware. And conversations should be documented. Would check with the clerkship on their procedure 🤷🏻♂️. Maybe needs to go through clerkship director?

I can’t count the number of peer-to-peer phone calls I have had to do — where I have other figure out where toe squeeze in this phone call (usually in between patients or during lunch) — where I essentially read my progress note to the insurance’s medical director.

Insurance wants to say that treatment decisions are between a patient and their doctor. But when they won’t pay for a treatment, they are inserting themselves right into the middle.
Provider: “We’re going to try and discharge you today if X, Y, and Z”
Non-English speaking patient: “No English”
Provider: “Well, I only speak English! So, as I was saying…”
Just…WHAT?!

Unfortunately the more “experience” I get in the practice of medicine in the current US system, the more I feel like we have it wrong. Huge panels, short amount of time per visit, limited access, frequent PCP changes all make it difficult to provide lasting health changes.
Moment of weakness last night — or early this morning.
Got some Taco Bell at around 12:30 AM. Quick, fast, comfort food that left me feeling regret.
