Clinical deterioration is an underappreciated risk of ED boarding.
In a multi-center study of >173,000 admissions, nearly 1 in 25 ED boarders admitted to the floor experienced early clinical deterioration requiring an admission to an advanced level of care, with almost half occurring before leaving the ED. Each additional hour of boarding was independently associated with an increased risk of deterioration, and deterioration carried substantially higher 28-day mortality.
Identifying high-risk patients early may prevent avoidable escalations in care and patient harm.

@shepski79 In order for a drug to enter ones system, say via skin, it needs help (a transdermal patch). The patches contain ingredients making the drug easier to pass through the skin. Also the adhesive holds the substance tight against the skin. Think about it.....

“We received numerous calls reporting a fire in this building. There were several calls saying there were people trapped. Upon arrival, they gave a 10-75, which was a confirmed fire. Captain Anthony Harper of Engine 163 was the first officer on scene. He was met by a civilian who brought him to the entrance door that was on the left hand side of the building, saying that there was a baby in the basement. Captain Harper went down to the basement. It was a very heavy smoke condition. He found the baby on the bottom bunk of a bunk bed, and it was towards the back of the house. He saw that there was a window right there. In an attempt to get to the baby to the cleanest air possible, as quickly as possible, instead of going back through the apartment that he had just come through, he began to pass the baby just out into the fresh air, not knowing that Firefighter Ryan Smith was on the outside from 86 Truck. He was assigned the outside vent position and was conducting his normal operations and had been in the back of the building when he saw the baby start to come out the window, he immediately took the baby, brought the baby to EMS resources that were already on scene, directly to the ambulance. The ambulance worked on the baby here at the scene to stabilize, and then transported the baby to Staten Island University North Hospital,” said Deputy Chief John Russell at the scene of a 2-alarm fire at 267 Ada Drive on Staten Island.

FDNY Paramedics Daniel Fennell and Michael Strobel quickly administered hydroxocobalamin, a lifesaving treatment which is used to treat smoke inhalation. Four others were injured and transported in stable condition to the hospital.

FDNY Fire Marshals are investigating the cause.
