What a great question. The reason @costplusdrugs doesn’t carry most brand meds is because the big PBMs threaten to de-emphasize or remove from their formularies, not just the drugs they sell to us, BUT ALL THE DRUGS THE SELL.

So Pharma is afraid to work with us. They risk losing billions in sales.

Why are the PBMs blocking them from working with a company that has only been shipping 3 yrs? Because they know if we net out the rebates and only mark up the brand meds 15 pct, their pricing will look outrageous. Because it is.

I’ll use elliquis as an example because o have taken it. The list price is about $600. If we got a rebate that was lower than the big PBMs, we would sell it for under $300 cash price.

They know employers will finally ask to see their net pricing on branded meds. Something they can’t see right now.

They know what we did to specialty generics pricing, where we charge hundreds or thousands less PER MONTH less than the cash and post discount amounts they charge.

And they know we are doing the same thing to them on biosimilars, right now. (Biosimilars are just generic meds that are injectable )

This is exactly why formularies need to be segregated from PBMs. If they are, net pricing will be used. Pharma could sell to use without fear , and the cash price for companies and patients , of many brand meds would fall by more than half.
