肝胆膵外科という沼

「なぜ肝胆膵外科なんですか？」

何度も聞かれた。

正直に言うと、合理的な理由だけでは説明できない。

もっと楽な道はある。

もっと効率よく成果を出せる分野もある。

夜中に呼ばれることも少なく、
休日を失うことも少なく、
精神的にも肉体的にも消耗しない働き方はいくらでもある。

それでも肝胆膵外科を選ぶ人がいる。

なぜなのか。

肝胆膵外科の手術は難しい。

難しいという言葉では足りない。

肝臓は巨大な血管の塊であり、膵臓は少し機嫌を損���るだけで術者を地獄へ連れていく。

昨日まで当たり前だった術式が、今日は通用しない。

教科書通りの症例などほとんど存在しない。

術前に何時間も画像を眺め、頭の中で何度も手術をシミュレーションし、それでも予想外のことが起きる。

そして手術室で問われる。

「お前はどうする？」と。

肝胆膵外科の魅力は、
メスを持つことではない。

考えることにある。

目の前の解剖を理解し、血流を読み腫瘍の振る舞いを予測し、術後の未来を想像する。

外科でありながら、どこか将棋やチェスに近い。

一手間違えれば勝負が変わる。

だが、正しく積み重ねれば道は開ける。

肝切除の瞬間が好きだ。

肝実質を進み、
門脈枝が現れ、
肝静脈が見え、
最後に標本が離れる。

その瞬間だけは、何度経験しても特別だ。

巨大な肝臓の中から、自分が思い描いた通りのラインが現れる。

画像の中でしか存在しなかった立体構造が、
目の前で現実になる。

あれは手術というより、答え合わせに近い。

膵頭十二指腸切除も好きだ。

もちろん楽しいわけではない。

むしろ苦しい。

だが、苦しいからこそ面白い。

切除が終わっても戦いは続く。

再建がある。

縫合がある。

ドレーンがある。

術後管理がある。

手術が終わった瞬間に半分しか終わっていない。

だからこそ、最後まで患者を見届ける覚悟が必要になる。

肝胆膵外科医は、成功した手術より失敗した手術を覚えている。

救えなかった患者。

防げなかった合併症。

もっと良い方法があったのではないかと思う夜。

そういう記憶が積み重なって、少しずつ外科医になる。

技術だけではない。

経験だけでもない。

責任を引き受け続けること。

それが肝胆膵外科医なのだと思う。

それでも、患者さんが元気に外来へ来てくれた日。

術後CTで再��がないと確認できた日。

「先生、旅行に行ってきました」

そう言われた日。

その一言で、
今までの苦労の多くが報われる。

たぶん外科医は単純だ。

患者さんの笑顔に弱い。

肝胆膵外科は、効率だけを求める人には向いていない。

コスパも良くない。

タイパはもっと悪い。

習得には何十年もかかる。

一人前になったと思った頃には、また新しい壁が現れる。

終わりがない。

だからこそ面白い。

肝胆膵外科は、人生を少しずつ削りながら、誰かの人生を少しだけ延ばす仕事だ。

割に合わないかもしれない。

非効率かもしれない。

それでも今日も誰かが手術室へ向かう。

術前画像を見ながら、頭の中で何度も血管を追い、切離ラインを描き、成功を祈りながら。

そしてまた次の朝、手術室の扉を開ける。

たぶん好きなのだ。

肝臓も。

胆道も。

膵臓も。

そして、このどうしようもなく難しい世界そのものが。
