•Redifferentiation restores RAl avidity using short-course MAPK pathway inhibitors (4-6 weeks), offering a fundamentally different strategy from long-term kinase inhibitor therapy - limited drug exposure, fewer cumulative toxicities, and potential for durable responses in selected cases.
•RAS-mutant and follicular-phenotype tumors are the best candidates - they retain more thyroid differentiation gene expression. BRAF V600E tumors respond less consistently but benefit from combined BRAF + MEK inhibition over MEK alone.
•Standardized protocols, prospective registries, and collaborative trials are essential to guide the appropriate candidate selection and obtain long-term safety outcomes.
