Most know how unsexy problems ruin lives quietly. Like, premature ejaculation is huge, under treated, and systemically downplayed. No FDA approved meds today, none. So we backed John Boghossian & his team at @KadenceBio a few years ago because fixing real life problems beats everything else.
Let’s talk about a category everyone giggles about, quietly googles in incognito, and then pretends doesn’t exist at the doctor’s office🤦🏼♂️. Premature ejaculation, yup. The punchline that isn’t funny anymore when it’s your relationship, your anxiety, your partner’s frustration. This isn’t niche at all, current estimates range from “at least 1 in 20 men” to “a big chunk of men depending on medical definition,” and Kadence’s own analysis pegs the global affected population at 100M+ men, plus their partners. That’s not a meme, far from it. That’s a market unfortunately and a public health problem hiding, growing in plain sight.
Here’s the crazy part, there is still no FDA approved medication for premature ejaculation in the U.S. Read that again. Doctors reach for off label SSRIs, tricyclics, topical numbing sprays, workarounds with awful side effects, and zero on label clarity. Dapoxetine, built specifically for PE, is approved abroad but not in the U.S. That regulatory vacuum is a big reason the problem still lingers.
The system we live in also downplays it because stigma is cheap and silence costs nothing. Men often don’t bring it up; physicians often don’t ask; and partners shoulder the fallout. The research is blunt, PE is linked to anxiety, depression, lower or no self esteem, and relationship strain and partners who have PE report lower relationship and sexual satisfaction. So the quiet suffering scales.
Now layer in the incentives. Regulators (rightly) want high safety bars for a non life threatening condition. Big companies prefer categories with cleaner narratives and historic wins. ED got Viagra and a spicy ending. PE got off label hacks and awkward silence, facts. Dapoxetine helped, but real world drop offs and side effects kept it from becoming the poster child. The result? ‘Unsexy’ aka, investors ducked the homework narrative.
At Ayuh, we fund reality. A few years ago we backed John & his team at Kadence Bio because they’re building the first credible on demand pathway to an FDA approved PE therapy. Their lead, KH-001, completed Phase I; Phase II is planned. The design goal is simple, fast onset, strong effect, better tolerability, taken when needed.
Stigma makes serious problems look small. Data makes them obvious. And building turns obvious into inevitable. We’ve said for years that brain health is infrastructure; sexual health sits squarely inside that. That’s why we partnered with Kadence on a problem most investors label “awkward.” Awkward doesn’t scare us.
