New article and podcast episode
With advancements in technology, there are now multiple laboratory tests that provide more accurate and useful information than LDL-C, HbA1c, or CGMs. Despite these improvements in testing, awareness remains limited.
HbA1c is the most commonly used tool for monitoring blood glucose control and diabetes. Importantly, there are several biological factors that may influence the reported value, for which HbA1c may not be a reliable or accurate test in all subjects.

In this post, I discuss several recognized instances where Hemoglobin A1c is elevated in the absence of insulin resistance or blood glucose dysregulation. This includes normal variations in red blood cell lifespan, as well as the concept of “exercise-induced insulin resistance”…

Evidence suggests that insulin resistance is the strongest risk factor for premature cardiovascular disease and cardiovascular disease at any age. Meanwhile, HbA1c does not adequately capture this risk. LPIR deserves greater awareness.

10. Clinical trials have demonstrated measurable and rapid improvements in MetS and LDL-C by replacing highly processed carbohydrates with higher quality starches. These health benefits can be achieved without a reduction in calories or a reduction in carbohydrates consumed.

This perspective is to acknowledge LDL-C/ApoB as one of many modifiable risk factors regarding ASCVD. Notably, there are several additional risk factors that appear to be stronger predictors of ASCVD than LDL/ApoB.

9. While many recommend limiting the consumption of dietary saturated fat for the sake of lowering LDL-C/ApoB, this dietary intervention has no meaningful impact on improving insulin resistance, hypertension, triglycerides, or the incidence of cancer.
