Orthostatic intolerance is a broad term that refers to a spectrum of dysautonomic disorders, including Postural Orthostatic Tachycardia Syndrome (POTS) and Orthostatic Hypotension (OH). A key feature of orthostatic intolerance is the appearance of symptoms upon standing.
Postural orthostatic tachycardia syndrome (POTS) is defined as a persistent increase in heart rate of 30 beats per minute (40 bpm in children) or more after a 10-minute change from supine to standing or sitting in the absence of OH. A heart rate of 120 bpm is commonly observed in POTS patients after repositioning. POTS primarily affects women of childbearing age. Patients with POTS and OH report debilitating symptoms including dizziness, tachycardia, presyncope, nausea, headache, difficulty concentrating, and memory problems.
An association between autoimmune diseases and orthostatic intolerance has been observed. Cases of patients with autoimmune OH have been published. In addition, about 20% of POTS patients have been diagnosed with commobid autoimmune disease (Hashimoto's thyroiditis, celiac disease, Sjogren's syndrome, rheumatoid arthritis, systemic lupus erythematosus, etc.).
The diagnosis of orthostasis intolerance is used:
1. Questionnaires (COMPASS-31, Malmo POTS score);
2. Passive orthostasis test;
3. In the absence of a passive orthostasis test, the NASA lean test can be performed, which is performed using a blood pressure monitor and pulse oximeter. During this test, how the patient's well-being and vital signs change after standing up and leaning their shoulders against the wall is monitored.
4. Electrocardiogram (ECG) and Blood tests to rule out co-morbidities.

If your mast cells are FLUSHING your skin or causing rashes on the outside - just imagine what is happening on the inside.
Mast cells are immune cells. They are the 'first responders' of the immune system and help protect us from pathogens, allergens, and environmental triggers.
Need an MCAS primer? This is a great powerpoint presentation that is directed towards 'docs'. It might even be a great resource to send to your doctor if they are not up on the latest links between mast cell dysfunction and #LongCovid and #ME. MCAS stands for mast cell activation disorder. You might not have MCAS itself, but you probably have dysfunctional 'mast cells'.
Mast cell dysfunction in general is extremely common in #ME and #LC and might even be part of the disease conditions itself. It is important to realize that mast cells are just a type of 'immune cells' - we all have them.
They are located in areas like the skin, the respiratory system, the GI track, the brain, and in connective tissue especially around nerves, and in the urogenital track. Unlike other immune cells, they are generally not floating around in the blood stream. Mast cells come from bone marrow and migrate into the tissue and stay there.
This is why mast cell symptoms are so stable. For example, if flushing is a major sign of mast cell dysfunction for you it will probably always be one of the main sites where you notice a mast cell reaction. The same goes for GI track (e.g. causing inflammation) or migraine/headache if the brain is a major site for you. Those with mast cell dysfunction usually notice greater symptoms in some systems (such as skin, GI, brain - you dont have to show symptoms is every bodily system for mast cell dysfunction to be playing a role).
Think about the visible signs of mast cell dysfunction in the skin. For example, you get FLUSHING or rash or itchiness. If this is happening on the outside, something similar is likely happening on the inside (such as the GI tract) you just cant 'see' it.
Mast cells are in greater density in tissue that comes into contact with the outside world. As immune cells they are guardians for pathogens and allergens and other dangerous substances. They are often associated with mucous membranes.
How are mast cells involved in neuroinflammation?
In the brain mast cells are located near the BBB (blood-brain-barrier) and in the meninges (the protective barrier covering the brain and spinal cord and the perivascular spaces near blood vessels).
Mast cells can also be found in the hypothalamus, thalamus, and the pituitary gland. They play a significant role in neuroinflammation and neuroimmune regulation by interacting with neurons and glial cells, influencing the blood-brain barrier (BBB), and modulating neurotransmitter levels. Their activation can affect processes like cognition, stress response, and the development of neurodegenerative diseases.
Most people do not realize that mast cell dysfunction can be associated with headaches and migraines. Managing mast cell dysfunction is an important part of managing neuroinflammation as well as cognitive issues like brain fog.

Don't know how PubMed works, but today they finally listed our paper on #POTS after #Covid & #Covidvaccines from 2022: still relevant & important to note that while POTS after vaccination can happen, it happens more frequently after SARS-CoV-2 infection.
