It's an add on of Google forms. Create a form and press the addons, might have to download the add on, but it's called "appsheet" and it will auto create an app based off your form

@PenderSuper An app via Google forms. App let's you submit new forms, view spreadsheet. Easy to use quick to set up. Everyone that needs access can see it, instead of single excel doc. Add location as part of the form questions and the app will use your phones GPS.
