A 32 yo woman presents with headache and somnolence.
She is febrile to 102.6F.
Exam reveals neck stiffness and this rash…which, when your attending arrives, is absent #hatethat!

Advice for physicians and APPs at all levels of training: never interrupt another staff member’s engagement with a patient. If an RN, RT, PCA, OT, PT etc are in the room, let them do their job and come back later.

Critical teaching point regarding CD4 count interpretation in persons with HIV! 👇👇👇
- CD4 count is back-calculated from the total WBC count.
- Total WBC x %lymphocytes —> absolute lymphocyte count (ALC)
- ALC x %CD4 cells —> “CD4 count”

@raghavchawla52 It’s never too late, I taught myself how to cook in residency! My housemates will attest that my indian cooking went from inedible to finger licking good 😂
Here is a nice example, showing time series over 14 days with the most recent results at top. Note the sudden increase in MCV, the reduction in MCHC and the elevation in RDW, all c/w reticulocytosis (retics were indeed increased owing to a bleed).
Our attending helped us keep tempo of chest compressions during a code today by singing “stayin’ alive, stayin’ alive…ah ha ha”. Probably the coolest moment in my residency training so far 😄
