Over 2 years ago we set up a service to support elderly patients discharged from ED and AFAU. Today we received our STAR rating 97%!! We could not do this without our fantastic CNS and therapists in @LiftRph our Consultant Geriatricians and our @UnitRph colleagues #frailty ❤️❤️

Fantastic joint working with @HHC_LTHtr today. I highlighted a virtual ward patient needing input. Referred and promptly assessed with equipment/care provision. Also highlighted District Nurse input needed and immediately referred. First class service keeping patients safe.
