As we draw closer to ๐ณ๐๐๐๐๐ ๐ณ๐๐๐๐๐ 2.0 ๐ a number of the featuring boxers are helping each other prepare for their boutsโฆ
One of the biggest things athletes have taught me over the years is that insertional Achilles tendinopathy does not always tolerate endless heel drops well in the early stages.
Many athletes who had already spent months โ sometimes years โ in failed rehab programmes told me the same thing:
โThese exercises just keep aggravating it.โ
That consistent feedback changed the way I approached these cases clinically.
Heel drops absolutely still have a role in Achilles rehabilitation โ particularly later in the loading progression โ but insertional Achilles pathology is mechanically different from mid-portion tendinopathy, and excessive dorsiflexion loading too early can sometimes flare symptoms rather than improve them.
Over time, many athletes responded far better to progressive isometric holds combined with focused shockwave rehabilitation before progressing into heavier dynamic loading later.
The biggest lesson?
Listen to your athlete.
Too many athletes are still being pushed through exercises that consistently aggravate symptoms simply because that has historically been the standard protocol.
Pain during rehabilitation is not always a sign the tendon is adapting well. Sometimes it is the tendon telling you the loading strategy is too aggressive for that stage of healing.
After 11 years of treating Achilles tendons with focused shockwave rehabilitation, itโs been fascinating to watch the profession evolve. Itโs great to see more clinicians now using focused and radial shockwave regularly.
What would be even better is seeing wider use of focused shockwave for chronic load-related injuries that continue to fail traditional rehabilitation approaches.
This 56-year-old ultramarathon runner developed severe osteitis pubis after repeated ultra-endurance loading. What began as โgroin painโ progressed into major functional limitation.
By the time he presented:
โข Walking was limited to ~2 miles
โข Coughing and sneezing caused sharp pubic/adductor pain
โข Turning in bed was severely painful
โข Getting in and out of the car hurt
โข He had to physically hold his legs together to roll in bed
โข He could not tolerate a simple adductor squeeze test
โข Right leg strength and stability had significantly deteriorated
Despite months of physiotherapy elsewhere, symptoms continued to worsen.
Eventually, the patient self-referred for MRI himself.
Imaging confirmed osteitis pubis with associated right hip labral pathology.
After researching treatment options himself, he attended our clinic for combined Focused Shockwave Therapy (fESWT) and EMTT alongside progressive rehabilitation and strict load management.
Importantly, because of the significant delay in diagnosis, imaging and targeted treatment, this case required substantially more focused Shockwave and EMTT sessions than would normally be expected in earlier-stage presentations. By the time treatment began, the patient was already in a severe and highly irritable state with major functional limitation.
Seven months later:
โข Pain-free coughing/sneezing
โข Pain-free bed turning
โข Pain-free car transfers
โข Walking 12+ miles without after-effects
โข Significant restoration of strength and function
Importantly โ he has not rushed back to running. Recovery from bone-stress pathology requires patience, progressive loading and respect for tissue healing.
Cases like this highlight how easily osteitis pubis can be missed in endurance athletes when early imaging is delayed โ and how prolonged diagnostic delay may significantly extend rehabilitation timelines.
