The evening of Jan 20th, 1991. Steve Waugh was dismissed for 60 on the 2nd day of a Sheffield Shield match against South Australia. It was, he felt, the final straw. That evening, he was at his fiancee, Lynette's place when the Australian coach Bobby Simpson called. He had been dropped for the next test against England.
Steve immediately drove over to his parent's place at Panania. When his mother Beverly opened the door, she immediately sensed something was wrong. She asked him what the matter was & he said he had been dropped.
She was in tears when Steve said it was all right, he wasn't too concerned. When his ma asked about his replacement, he said, 'he's right here' & pointed to his twin Mark, chatting with their dad Rodger.
Mark got 138 on debut & they went on to play 108 tests together.

Many findings can be seen with NPH—you may use 1 or more. But do you know how well your sign performs?

Here are how common NPH signs perform:

Evans index
SENS: 100
SPEC: 74

Cingulate sulcus sign:
SENS: 63
SPEC: 61

Callosal angle:
SENS: 49
SPEC: 78

Disproportionately enlarged subarachnoid spaces (DESH):
SENS: 94
SPEC: 41

Anteroposterior diameter of the lateral ventricle index:
SENS: 98
SPEC: 100

For the SINPHONI trial, a combination of Evans index > 0.3 & DESH had a 70-80% PPV for shunt responsiveness.

In this month’s @theAJNR, Leary et al decided not to pick & choose what to use & instead used an AI algorithm based on T2 & FLAIR images. They found they could get >80% sensivity & >80% specificity. This could be used as a screening tool in NPH!

Now you know the signs of NPH & how good they may or may not be. Hopefully, you can now set the bar high when it comes to hydrocephalus!

In 1986, 22-year-old flight attendant Neerja Bhanot was on duty during Pan Am Flight 73 when terrorists hijacked it.

One Indian-American passenger was swiftly killed by the hijackers, who then asked Bhanot to gather all passengers' passports to identify the Americans. Defying their orders, Bhanot hid 43 American passengers' passports, some under seats and others in the trash.

After 17 grueling hours, the terrorists started setting off explosives and shooting. Bhanot, instead of escaping, opened the airplane doors and guided passengers to safety. In the process, she lost her life while protecting three American children from gunfire.

One of the children she shielded, only seven at the time, later became an airline captain. He credited Bhanot as his inspiration.
