.@gabegutierrez was given exclusive access as a UPS truck delivered Covid-19 vaccines to The Ohio State University Wexner Medical Center, where the staff was among the first in the U.S. to receive the vaccine.
Live now on https://t.co/90BcIYqI19: Dr. Mercy Dickson, one of the first vaccine recipients at @OSUWexMed. She is a third-year emergency room resident.
@OhioACEP @ACEPNow @OSUWexMed If you are having symptoms that you are worried could be an emergency it is OKAY to call 911 or come to the ER. If you think it could be COVID19 related, be sure to tell the EMS / ED staff. #ERhere4U https://t.co/QW4m6skqX0
Lesson in litigation- beware of “he said she said” game with consultants- document details discussed pertinent to decision making and proposed plan #EEM19#essentialsofem#emergencymedicine
Lessons in litigation in EM- don’t send a test you don’t want answer to! Bandemia in CBC may bite you in the ass- so document and awknowledge it! Yes I considered it... #EEM19#essentialsofem#emergencymedicine
Action items- document pertinent positive/ negative (ex for headache/ fever- no AMS/ no neck stiffness/ kid eating food/ acting normally) and document stable at DC! DOCUMENT WELL! #essentialsofem#EEM19#emergencymedicine
Other tips- Every 6u prbc give an amp of calcium chloride; send fibrinogen level when massively transfusing; give PCC if patient on warfarin; fentanyl for pain control #weingart#EEM19#essentialsofem
Give platelets and TXA (if no contraindication)- let drip during massive transfusion...then decide what MAP should be - minimal normotension (65 MAP and perfusing); 80 MAP in TBI (no great evidence); if ice cold extremity- not perfusing, increase MAP goal #weingart#EEM19
Ear foreign body? Irrigation is great, but not for ears with batteries, bugs or organic material. Batteries are at risk for coagulation necrosis, and bugs and other organic material are likely to swell, disintegrate and become more unmanageable. #EEM19@BeharSolomon
Pressor use should be triggered EARLY- low dose norepi- increases venous return for fluid given or endogenous fluid...consider in first BP drop; might be able to take off pressors in ED! #EEM19#essentialsofem#emergencymedicine
What triggers your critical care sepsis bolus? 30cc/kg bolus- hypotension, lactate >4, actual vs ideal weight...can the patient tolerate without harm? Healthy patient, healthy heart vs 90 year old with EF 20%...#weingart#EEM19#essentialsofem#emergencymedicine
Provocative testing in low risk chest pain does not improve outcomes at 1 month or 1 year.
Stop doing provocative testing in low risk patients! The specificity of these test are 75-80%! Too many false positives!
