Just starting an interesting day on the T-MACS Network organised by @HealthInnovMcr to spread the word about a algorithm that still hasn't reached its full potential.

T-MACS is based on logistic regression. It isn't only great at rule out but also does a great job at ruling patients in as well.

Machine learning can and hopefully will prepare TMACS to the future and make it even better. Great work by @Reynard_EM in progress.

T-MACS stratification in ~1500 patients at MRI identifying 61% patients as very low or low risk with excellent sensitivity.

2018 will be a big year for the #MACS_Project:
(1) BEST study results
(2) PRESTO study (pre-hospital) begins
(3) Regional implementation
(4) Some exciting new angles for the decision aids yet to come!
TMACS has been used at @MFTnhs since June 2016, & is now being rolled out across Greater Manchester as a @healthinnovmcr exemplar project
We'll keep you up to date as the project progresses in 2018!
Originally, #TMACS and #MACSRule had only been shown to work with one assay: hs-troponin T
The amazing @patricia_vdberg put that right by validating both with a contemporary assay (Siemens cTnI Ultra)

#TMACS was externally validated by some of the greatest Emergency Med researchers in the world, from Australasia @louiseacullen @LastCardiology @KiwiskiNZ

For some, #HFABP was a barrier to using the #MACSRule. Labs aren't already running it. We refined the model, creating #TMACS: now the only biomarker needed is hs-troponin T

Patients who took part in the #MACSPilot told us what they thought of their participation. Very valuable insights for all emergency research from this qualitative piece, led by @patricia_vdberg

A pilot RCT comparing the #MACSRule to standard care showed the feasibility of a large trial. Significantly more safe discharges occurred in the intervention group (26% v 8%, OR 5.45)

One criticism was that the #MACSRule used an #HFABP assay that couldn't be run in the ED. @RandoxOfficial developed an automated assay to overcome that! It can be run on any lab analyzer
