Hiccups can signal phrenic nerve irritation from septic emboli or mediastinal infection. Cellulitis may be a portal of entry or a site of embolization, especially in IV drug use.
IE is a master of disguise—never dismiss persistent bacteremia or unexplained symptoms.
Always think endocarditis in patients with bacteremia and unusual complaints. Early cultures and echocardiography can be lifesaving.

Patient 64 presented with rapidly progressing memory loss and ataxia. A CT scan revealed dilated lateral ventricles, and laboratory tests showed a vitamin B12 deficiency. The differential diagnosis included normal pressure hydrocephalus (NPH) and subacute combined degeneration.
We performed a lumbar puncture (Opening pressure 32) and initiated IV Cyanocobalamin treatment. The patient subsequently showed clinical improvement. However, it remains unclear whether the improvement was due to the LP (supporting NPH) or the B12 administration (supporting subacute combined degeneration). 😆
Additionally, the patient had an equivocal syphilis serology, raising concern for neurosyphilis. However, CSF VDRL was negative.
