Again compounding mix ups, errors in conversion of units (mg to micrograms) and overdoses of up to 1000x the appropriate dose have occurred! Fortunately the ISMP has recommendations including standardization, double checks and education!

Drug dosing errors can happen in everyone, but children have an added complexity of often needing weight based dosing, and having to compound liquids. We always need to be vigilante, but be extra cognizant of the children!

Survey of pharmacies found that up to 9 different concentrations may be available for a single drug!! This can set up caregivers for errors when we talk about volumes vs drug doses!!

Neurocritcially ill patients can sometimes have augmented renal clearance (ARC) that cause more rapid clearance of drugs. Luckily their are ARC prediction scores available!

Do you think about how hypothermia can slow metabolism of medications in critically ill neurological patients? It can't affect it a lot! (Increases AUC of phenytoin by 180%)!
