Whole-body #bone#scintigraphy plays an important role in evaluating bone lesions of #SAPHO syndrome and may especially reveal the characteristic appearance in the sternocostoclavicular region, known as the bullhead sign, which was first describen by Kasperczyk and Freyschmidt in 1994. This sign is defined as the manubrium sterni representing the upper skull and the inflamed sternoclavicular junctions with the adjacent claviculae forming the horns.
Athough the bullhead sign is specific for the SAPHO syndrome, it is not entirely sensitive and recent studies showed that its frequency was only 22.9%, contrasting with previous articles that showed a much higher percentage (87.8%) possibly because of the selection bias as most of their patients had sternocostoclavicular hyperostosis.
