ESR has in fact been shown to have limited diagnostic accuracy, even for experienced endourologists. Perhaps specific training with direct feedback or the combination of clinical information + visual appearance could improve the accuracy, but this still needs to be proven 2/2
Even though ESR can have a role in the analysis process (e.g. what parts of the stone to send for analysis to avoid sampling bias), I agree that formal stone analysis should still be the corner stone in our daily practice as we are not as good at it as we’d like to think. 1/2
