Prospective randomised study on the use of fibrinogen concentrate and PCC in patients admitted to hospital with massive haemorrhage. Only the plan presented at #ASH20 as the study opened in December 2020. Looking forward to the results in 2-3 years time.

That’s right. SAVER will randomize VTE patients to rosuvastatin or placebo on top of standard treatment to prevent VTE recurrence. Almost ready to start in Canada, soon in Europe with the help of @INVENT_VTE and national networks.

Reality check: Germany (pop 83million) running 200-300 cases per day and sudden outbreak is 1000 cases from a factory. UK (pop 66million) running 1200-1500 cases per day. Like an equivalent big outbreak everyday in the UK.

1. This virus is too dangerous to let spread through a population unchecked. Not only bc of health services capacity & deaths in elderly/vulnerable groups, but also because of the morbidity it causes. Not the flu but a multi-system disease.

Renal replacement therapy in severe #COVID-19 is difficult- the ++prothrombotic state ➡️ premature clots in circuit ➡️ ⬆️ changing of lines ➡️ shortages

Consensus management today = renal adjusted full dose LMWH (Europe) or UFH infusion (US) in pt & usual anticoag in circuit

My hosp’s transformation to a giant critical care unit with 100s of beds for #COVIDー19 is near complete.
We see resp failure + AKI +mild hepatitis & PROFOUND inflammatory state - ferritins in 1000s, CRPs in 100s, D-dimers ++, fibrinogen >10g/l
BUT norm plt, PT/APTT i.e no DIC
