Barnett’s sign (scleroderma neck sign)

A clinical feature of systemic sclerosis (scleroderma).

It is characterized by the visible and palpable tightening and ridging of the skin of the neck when a patient hyperextends their head backward.

*Barnett first described it as a highly prevalent and confirmatory bedside sign, with some studies finding it in over 90% of scleroderma patients.

**It is typically absent in individuals with primary Raynaud's phenomenon or other non-connective tissue controls, making it a useful differentiator early on.

***While useful, it is highly subjective and not included in the official, widely recognized ACR/EULAR diagnostic criteria for systemic sclerosis.

In just the past few days, more than 10 applicants have reached out to me about the ongoing MSPE upload issues on MyIntealth. This isn’t an isolated problem, hundreds of IMG residency applicants are stuck because their schools and applicants themselves can’t upload the MSPE.

When they contact @Intealth, all they get is a generic email response, while the delays are directly affecting ECFMG certification timelines and adding enormous stress during an already challenging process.

I strongly urge Intealth to address and resolve this issue immediately, the future of countless IMG applicants is at stake.
