If everyone had @surgeonretina available, then for sure a PPV and scleral fixated IOL. However, with careful use of OVD and fluidic mgmt, this could be managed via a clear corneal incision approach, with a limited anterior vitrectomy after nucleus sequestration and removal.
Even in a post EAGLE and ZAP trial world, laser peripheral iridotomy is still valuable in managing angle closure spectrum disease. Where do you place your LPI? #glaucoma#ophthalmology
