💡Could Steroids Prevent Pacemakers After TAVI? GLUCO-TAVI Trial #ESCCongess #ESC2025

🎯 Objective
Test whether a short course of glucocorticoids can reduce cardiac conduction defects (CCD) and permanent pacemaker implantation (PPI) after TAVI.

💉 Method
Design: First PROBE-design randomised trial
Population: 100 patients with severe aortic stenosis

Intervention: Standard care vs Methylprednisolone 7 mg/kg 1h pre-TAVI + prednisone 15 mg/12 h for 5 days
Follow-up: ECG & CRP post-TAVI, at 1 month, and 1 year

📊 Results:
⚡PPI at 1 year: 12% (steroids) vs 18% (control) → OR 0.62 (NS). New LBBB: 38% vs 34% (NS)

🎇 Inflammation: CRP rise post-TAVI significantly lower with steroids (+0.14 vs +1.62 mg/L, p < 0.001)

☠️ Safety: No increase in mortality or major bleeding

🚀 Why this matters
Although non-significant, even a modest reduction in PPI could mean fewer cases of pacing-induced heart failure, lower healthcare costs, and better quality of life.
