My concern is for the physician who orders the labs as recommended by SMFM, but is not given guidance on their interpretation. Non-fasting bile acids of 12 -> weekly antenatal testing with consideration for delivery at 37-39 weeks? Elevated LFTs with normal BA?

2) UDCA now with questionable association with changes in BA levels https://t.co/KUWkGC30aH and no improved outcomes
3) Reasonable to perform serial BA levels and deliver based on HIGHEST, not MOST RECENT level??

Lessons gleaned from this:

1.) high risk of recurrence in ICP (~70%)
2.) Serial lab assessment can be critical
3.) Optimal dx, surveillance and delivery timing for ICP unknown.
