It was a great pleasure to participate in this scientific meeting held in Neum through the collaboration of IURES, the Balkan Urology Association, and the Urology Section of the Serbian Medical Society.

With nearly 150 participants from 9 countries, the congress provided an excellent opportunity for both scientific exchange and international friendship. Together with our delegation from Türkiye, we shared our academic and surgical experience in urology while conducting hands-on courses on 3D-printed models in Robotic/Laparoscopic Surgery, Stone Surgery, and Sacral Neuromodulation.

I believe these growing academic collaborations across the Balkans will lead to even stronger projects in the future.

Anatomi doktora eğitimimle birlikte başlayan bu yolculukta, 16 yıldır ürolojide kadavra modeli üzerinden birçok cerrahi eğitim organize ettik. Bu süreçte eğitici olarak yer aldım; onlarca makale ve bildiri hazırladık, yaklaşık 200 organizasyonda aktif görev aldım.

Şunu çok net görüyorum: Cerrahiyi öğrenmek isteyen, özellikle genç meslektaşlarım; önce “cerrahi anatomi”yi derinlemesine anlamak, ardından cerrahi teknikleri adım adım görmek, uygulamak ve bizzat deneyimlemek istiyor. Bu kursların her dakikası bilgi ve beceriyi artırmaya odaklı olduğu için, çoğu zaman birçok kongreden daha fazla ilgi görüyor.

Bugün de IURES FuncURO ve IURES Andro özel ilgi grupları olarak Akdeniz Üniversitesi Anatomi Laboratuvarı’nda gerçekleştirdiğimiz kursumuzda, 15 farklı şehirden 30 katılımcıyı ağırladık. Büyük bir dikkat ve motivasyonla dinlediler; yorulmadan, sıkılmadan diseksiyon yaptılar. Bu kez onlar uyguladı, biz mentorluk yaptık. Herkes o diseksiyonu kendi elleriyle hissetti.

IURES çatısı altında “Kadavra Modelinde Ürolojik Cerrahi Okulu” projemizi de çok yakında hayata geçiriyoruz.

Sakral nöromodülasyon cerrahisine yönelik, kişiye özel 3D modelleme ve cerrahi planlama algoritmalarını içeren yöntemimiz için patent başvurumuz gerçekleştirilmiştir.

Başvuru sahibi Hacettepe Üniversitesi olup, buluş sahibi olarak bu sürecin bir parçası olmak benim için büyük bir gurur ve mutluluk.

Yaklaşık 10 yıldır sakral nöromodülasyon cerrahisi ile aktif olarak ilgileniyor; bu alanda yalnızca klinik uygulamalar değil, aynı zamanda cerrahi tekniklerin geliştirilmesine yönelik eğitim ve araştırma çalışmaları da yürütüyorum.

Bugün geldiğimiz nokta, aslında uzun bir yolculuğun sadece başlangıcı…

Henüz yolun başındayız.
Umarım bu çalışma, cerrahi pratiğe ve hastalarımıza katkı sağlayacak yenilikçi bir adımın başlangıcı olur. ✨

🔬 Importance of the S3 Foramen in Sacral Neuromodulation Surgery

• Identifying the location of the S3 foramen alone is not sufficient during SNM 1st step surgery.

• The three-dimensional modeling bony measurements of the S3 foramen are equally important:
• Z axis: Oblique bony depth
• X axis: Foraminal width
• Y axis: Foraminal height

• Measurements obtained through 3D modeling play a critical role, particularly for the proper and stable placement of the introducer sheath.

• In sacrum with a narrow foramen, the introducer sheath may become compressed within the foramen, making electrode passage more difficult.

• Therefore, a foraminal diameter of ≥5 mm provides a more comfortable and safer surgical access.

• In this patient, the S3 foraminal bony depth was measured as 8 mm, and an anatomical electrode placement was achieved by positioning the introducer sheath deeper.

Sacral Neuromodulation | Two Challenging Cases

3D Modeling–Assisted Surgical Planning

Sacral neuromodulation is an advanced neuro-urological surgical treatment for refractory voiding dysfunctions. In selected patients, 3D anatomical modeling is used for surgical planning to improve accuracy and outcomes.

📌 Case 1
A patient with prior radical colon surgery, colostomy, chemotherapy, and radiotherapy developed severe voiding dysfunction.
Stage 1 sacral neuromodulation was performed. Effective reflexes were obtained at 1 mAmp, and the patient reported easier voiding during the test phase. Evaluation for permanent implantation is planned within 1–3 weeks.

📌 Case 2
Following trauma, a patient with a recent permanent implant presented with leg stimulation due to a 3 mm lead displacement.
Lead revision was performed, and effective responses on all four electrodes at 0.5 mAmp were achieved.

🔎 Take-home message
Sacral neuromodulation is a precision-based surgery.
Proper patient selection and 3D planning allow safe and effective treatment—even in challenging cases.

Sacral Neuromodulation – Stage 2 (Permanent Pulse Generator Implantation):
In patients who demonstrate a clinically meaningful response during the first stage (test stimulation), implantation of the permanent pulse generator is performed under local anesthesia. During this stage, the previously implanted electrode is preserved and connected to the permanent generator. Intraoperative and postoperative impedance checks are of critical importance to confirm the safe and effective functioning of the system. Appropriate impedance values represent a key parameter for long-term therapeutic success and sustained device performance.

In a 32-year-old male patient, retrograde urethrography demonstrated a proximal penile urethral diverticulum with an associated penoscrotal fistulous tract.

Based on these findings, the patient underwent complete excision of the urethral diverticulum and fistula, followed by urethroplasty with dartos flap interposition.

The surgical objective was total removal of the pathological segment, restoration of urethral continuity, and prevention of fistula recurrence.

This case highlights the critical role of retrograde urethrography in surgical planning and the importance of tissue-supported reconstruction in complex urethral pathology.

Between October 17 and 19, 2025, IURES participated with a seven-member delegation in the Macedonian Urology Congress & Balkan Urology Congress, held in Struga, with the objective of contributing academically and fostering collaboration with colleagues from the region.

The congress was marked by high attendance, a comprehensive and high-quality scientific program, constructive discussions, and rich social activities, making it a highly productive and engaging event.

During the upcoming IURES 2025 Congress, which will take place on November 6–9, 2025, the Balkan Urological Association will actively contribute to the International Urological Societies Session with 25 participants.

Guided by the motto “The Future Together,” IURES continues to advance its mission of representing Turkish Urology on the international stage through impactful academic collaboration and scientific excellence.

Aydın Adnan Menderes Üniversitesi’nde Visiting Profesör olarak, Sakral Nöromodülasyon Cerrahisi üzerine tam gün süren kapsamlı bir eğitim programı gerçekleştirdik.

Eğitim kapsamında;

🔧 Ameliyatta kullanılan malzemeler,
📚 Güncel literatür ve kılavuz değerlendirmeleri,
🎥 Ve bugüne kadar gerçekleştirdiğim vakalardan seçilmiş ameliyat videoları üzerinden detaylı teorik paylaşımlarda bulundum.

Ardından, iki farklı endikasyona sahip iki hastada canlı cerrahiler gerçekleştirdik:
👨‍⚕️ İdiopatik üriner retansiyon tanılı bir erkek hasta
👩‍⚕️ Refrakter aşırı aktif mesane tanılı bir kadın hasta

Anabilim Dalı’nın değerli öğretim üyeleri, araştırma görevlileri ve farklı hastanelerden katılım sağlayan üroloji uzmanlarının yoğun ilgisi ve katkısı, bu eğitimi çok daha anlamlı hale getirdi.

Aydın ilinde ilk kez uygulanan Sakral Nöromodülasyon Cerrahisi olması, bilginin ve deneyimin paylaşımı açısından büyük bir adım oldu.

🔹 SNM Revision Surgery 🔹

In this case, I removed a previously implanted electrode from an outside center and placed a new tined lead into the S3 foramen.
📌 The patient’s subcutaneous–sacrum distance was 2.1 cm, which made entering S3 at a 60° angle challenging.
📌 As a result, the first lead had a limited clinical effect.

✨ What made the difference this time: 3D surgical planning.
It allowed me to better understand the complex anatomy and approach the procedure more safely and effectively.

Revision cases in SNM are always more challenging than standard implantations. But with careful planning and patience, successful outcomes are possible. 🙌

👨‍⚕️ 66-year-old male patient with a long history of voiding difficulties, previously treated for benign prostatic hyperplasia with partial benefit. In recent months, he required first an indwelling catheter and later intermittent catheterization. Further evaluation revealed underactive bladder (detrusor underactivity).

Based on these findings, Sacral Neuromodulation (SNM) – Stage 1 was planned and performed.
🔹 The electrode was placed at the S3 foramen with appropriate angle, direction, and depth.
🔹 No reflexes were obtained on the right S3 side, while on the left S3, effective responses were achieved at <1 mAmp.
🔹 A permanent electrode was successfully implanted within just 2 minutes and 15 seconds.

💡 Importantly, the use of 3D surgical planning and modeling contributed significantly to the speed, safety, and precision of the procedure.

The patient’s clinical benefit will be closely monitored in the following weeks.

📌 Sacral neuromodulation is a neuro-urological surgical procedure that should be considered in carefully selected patients, offering advanced treatment options for bladder dysfunction.

Sakral Nöromodülasyon (Mesane Pili) 2. Aşaması: Kalıcı Pil Yerleştirilmesi

Lokal anestezi şartlarında uygulanan işlemde, pilin üzerinde yer alan giriş yerinden çentikli elektrotun kuru olduğuna emin olunduktan sonra yerleştirilir ve sıkıştırılır. Olası bir gevşeklik veya yerinde oynama ihtimali de kontrol edilir. Uygun olmayan ataçman işlemi, empedansın yüksek olması gibi sonuçlar doğurabilir. Bu durumda aynı işlem yeniden tekrarlanır.

Empedans, pil (+) ile 3-3-1-0 numaralı elektrotlar (-) arasındaki akım uyumunu gösterir. Kanama, pıhtı veya temas sorunları yüksek empedansa neden olup tedavideki başarısızlık nedenlerinden biri olabilir.

Unutulmaması gereken en önemli konu “doğru hasta seçimi” ve her hastada kişiye ve kliniğe özel bir planlama yapılmasıdır. Bu şekliyle mesane pili, işeme bozukluklarında etkili bir tedavi yöntemidir.
