This means that the difference between serum albumin and ascites albumin concentration is lowered. Nephrotic syndrome is an anomaly - it is not inflammatory, but it gives rise to a low SAAG due to a reduced serum albumin concentration (as albumin is lost in the urine).⁠⁠

It also becomes permeable to proteins, such as albumin, which would not normally leak from the intravascular compartment.

In any patient that is nil-by-mouth, it is important to remember that there are some critical drugs that cannot be omitted - these include Parkinson's drugs, antiepileptics, antiretrovirals and long-term steroids.
