Flail chest: When 2+ ribs break in 2+ spots, a "floating" segment detaches from the chest wall. Paradoxical breathing is a key sign—unlike normal chest expansion during inhalation, this segment moves inward, then outward on exhalation, due to pressure changes in the lungs.
