How do we calculate the time spent clearing up post-meeting commitment confusion or needing to discuss the same thing again and again (and again)?
We don't because we can't.
What can be calculated (and then just add a bunch of zeros to that number) is the cost of the actual booked meeting time. We consider the cost of consulting time before we ask for consulting time.
